We are so excited you have choosen to take the next steps in your faith and are getting baptized! This is a big deal! Please fill out this questionaire to help us get an understanding of your view of baptism and why you have choosen to be baptized. 

If you would like someone else who has been a big part of your faith journey to baptize you that is fantastic! All we ask is for someone who has been baptized by immersion.
